Dear Parents,

Your child was given a Public High School Application today. If you are interested in your child applying to a Public H.S. please complete pages 1 & 2. The application should be returned by Monday, November 10^th.

In filling out the STUDENT CHOICE PAGE it is very important that you list the schools in the order of your preference. You can list up to 12 schools. (BE SURE TO LIST BOTH THE PROGRAM CODE, PROGRAM NAME AND SCHOOL NAME). Students will only be accepted into ONE PUBLIC HIGH SCHOOL. If a student is accepted into more than one school, he or she will only receive notification of acceptance into his or her highest choice.

If your child took the Specialized H.S. exam, please color in the bubble next to Specialized High Schools in addition to listing the other high schools. If your child is auditioning for Fiorello LaGuardia H.S., please fill in the bubble for the particular program he or she will be applying and auditioning for. DO NOT WRITE THOSE SCHOOLS BELOW.

This is your only chance for your child to apply for a Public H.S. Please spend some time considering your choices. IF YOU ARE NOT SURE WHETHER OR NOT YOU WISH YOUR CHILD TO ATTEND A PUBLIC HS, PLEASE FILL IN THE FORM AND LIST SOME SCHOOLS. YOU CAN ALWAYS DECLINE THE ACCEPTANCE LATER ON.
